What to Bring


Please bring the following to the Acute Admission Process:


  • Photo ID of guardian
  • Insurance Card for the child
  • Custody papers, if applicable
  • Social Security number for the guardian
  • Social Security number for the patient
  • Social Security number, Date of Birth, Address and Phone Number of the person who carries the insurance (if different from guardian)
  • 4 changes of clothing for the child
  • 4 pair of socks
  • 4 pair of underwear (no g-strings or thongs)
  • 4 pair of pants or shorts (must be long shorts)
  • 4 shirts (no tank tops or spaghetti straps)
  • 1 pair of tennis shoes
  • 1 pair of flip flops
  • 2 outfits to sleep in (regular pajamas are fine or shorts, jogging pants and t-shirts)
  • 1 jacket (no hoods) – if needed
  • All medications

***DO NOT bring any personal hygiene items. We will provide all items needed.

***NO STRINGS OR BELTS on clothing.
