What to Bring

What to Bring to Laurel Oaks Behavioral Health Center

Please bring the following to the acute admission process:

  • Photo ID of guardian
  • Insurance card for the child
  • Custody papers, if applicable
  • Social Security number for the guardian
  • Social Security number for the patient
  • Social Security number, date of birth, address and phone number of the person who carries the insurance (if different from guardian)
  • Four changes of clothing for the child
  • Four pairs of socks (ankle socks only)
  • Four pairs of underwear (no g-strings or thongs)
  • Four pairs of pants or shorts (must be long shorts)
  • Four shirts (no tank tops or spaghetti straps)
  • One pair of tennis shoes (no shoe laces)
  • One pair of slide-on shoes
  • Two outfits to sleep in (regular pajamas are fine or shorts, jogging pants and t-shirts)
  • One jacket (no hoods) – if needed
  • All medications
  • DO NOT bring any personal hygiene items. We will provide all items needed.
  • NO STRINGS OR BELTS on clothing.
  • NO leggings or camouflage
